| Cover commencement | Cover commences on the first day of your employment with Phoenix ME Limited |
| Cover ceasing | Cover continues until you either leave the company’s employment or reach age 75. |
| Nomination of beneficiary | It is advisable to nominate a beneficiary for this benefit in the event of your death. You should also review your nomination of beneficiary on a regular basis.
